Our Certified Nursing Assistant is responsible for assisting in the delivery of patient care through the nursing process of assessment planning, implementation and evaluation. Assists with patient care and patients with ADL’s commensurate with his/her education and demonstrated competencies. CNA staff are also responsible for safely and effectively administering medication.

Qualifications and Educational Requirements

– High School Diploma or GED

– Valid and Current certification as a Nurse Aid in the State of Wisconsin

– Has a reliable means of transportation

– Basic health and TB screening (within last 6 months)

– Valid Wisconsin CBRF certifications in First Aid & Choking, Medication Administration (unless also a Med Tech) and Fire Safety

Required Work Experience

– Preferably one (1) year of employment as a Nurse Aide.

– Provides other services such as, but not limited to, washing wheelchairs and cleaning patient areas in compliance with infection control procedures.

– Contributes to the patient assessment and care plan.

– Makes routine rounds on each assigned patient and patient in accordance with established procedures.

– Promptly communicates any significant changes in behavior or condition to supervisor.

– Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Job Summary: The Prenatal Care Coordinator is responsible for recruiting and enrolling pregnant and/or new moms who receive state benefits in Milwaukee County.  Care Coordinators will perform initial intake assessments, provide resources, advocate, follow up and create achievable goals. Coordinators are also responsible for providing ongoing care coordination for families, as well as assisting with group and individual health and nutritional training sessions. This role is responsible for successfully implementing the Prenatal Care Coordination model, goals and desired outcomes. 

Job Duties/Responsibility: 

· Meet with clients in their home, or our office to complete potential client screenings and assessments.

· Identify needs, problems and develop a care plan to assist mom in becoming self-sufficient.

· Assist clients in developing a safe & healthy environment for themselves & their children

· Attend and actively participate in meetings and trainings

· Complete special projects as assigned, community events such as baby showers, back to school events, parties, etc.

· Complete and maintain all required paperwork, records, documents, etc.

· Perform other duties as assigned by Chief Operating Officer-Programs

Title: Care Coordinator – Crisis Services

Position Type: Regular Employment (Full-Time/Part-Time)

Job Summary: This position is responsible for providing intensive, short-term services to children and adolescents who present with thoughts, emotions and/or behaviors requiring additional skilled professional intervention to stabilize their current situation and prevent the need for a higher level of care.  This position is also responsible for providing individual skill development and enhancement services to support youth and young adults as they reach their recovery goals. The Crisis Stabilizer role serves youth at various stages of their recovery from mental health, substance use, and co-occurring needs by embracing the youth’s strengths to meet their identified needs. Other components may entail debriefing the crisis with the youth as well as teaching the youth and caregiver(s) the skills necessary to prevent crises in the future. Crisis Stabilization and Crisis Tracking

Job Duties/Responsibility: 

  • Act as a member of a team that provides services to youth and young adults with mental health, substance use, and co-occurring needs.
  • Build off the person’s strengths to meet their individualized recovery needs. All services will be person-centered and directed by the individual being served.
  • Provide crisis stabilization services, including individual skill development and enhancement services related to training in communication, interpersonal skills, problem solving, decision making, self-regulation, conflict resolution, and other specific needs identified in the Plan of Care.
  • Services also include training in daily living skills related to personal care, daily tasks, transportation, school, and accessing and connecting to community resources and services (including healthcare services) and other specific daily living skills identified in the Plan of Care.
  • Utilize various methods to serve people including but not limited to: on-call de-escalation, role modeling, monitoring, mentoring, supervision, assistance, and cuing.
  • Provide services in the community including homes and other community locations.
  • On-call 24 hours per day, 7 days per week, to provide de-escalation to youth and young adults in crisis to assist with self-regulation and problem-solving through challenging situations.
  • Responsible for maintaining flexibility in daily work schedule to adequately meet program needs.
  • Responsible for adhering to agency policies and procedures while carrying out daily responsibilities.
  • Perform other duties as assigned by Chief Operating Officer-Programs

  • An understanding of mental health and co-occurring needs and the ability to clearly communicate these their understanding to other individuals;

Title: Care Coordinator – Waukesha County

Reports to: Chief Operating Officer-Programs

Job Summary: This position is responsible for providing support to between 9 to 12 individuals at any one time and maintaining weekly contact by phone and/or in-person visits. This position involves the care and treatment of adults and youth diagnosed with a severe mental illness and/or substance use disorder. This position coordinates and/or delivers psychosocial rehabilitation and treatment services in the most natural and least restrictive manner to be delivered with reasonable promptness and build upon the natural supports available in the community.

Job Duties/Responsibility:

  • Formulate recovery focused client-centered treatment plans for adult and youth on his/her caseload.
  • Coordinate agency staff and community resources to support and implement client-centered treatment plan goals.
  • Provide individual skill development to adult and youth with serious and persistent mental illness and co-occurring disorders such as substance abuse.
  • Provide wrap-around services to clients including but not limited to; communication, interpersonal skills, problem solving, decision-making, self-regulation, conflict resolution, community life skills training, employment related skill development, school and IEP advocacy, symptom management and psychoeducation.
  • Follow recovery focused client-centered treatment plans for clients on his/her caseload.
  • Provide support and crisis intervention, as needed.
  • Maintain a treatment oriented environment and assure consistency in carrying out treatment objectives.
  • Support clients with taking medications as directed by the supervising physician.
  • Complete and coordinate appropriate and timely medical record documentation, which may include assessments, follow-up contacts, crisis plans, discharge summaries, demographic, insurance, and administrative information.
  • Provide oral and written communication that is appropriate to the task and in accordance with agency, DHS 36 and Medicaid standards.
  • Participate in staff meetings and in-service trainings, as required.
  • Maintain flexible work schedule to adequately meet program needs.
  • Perform other duties as assigned, including responding to an emergency event.

Knowledge, Skills and Abilities:

  • Knowledge of Chapters 36 and Mental Health.
  • Ability to respond to crisis situations and make appropriate assessments as to a resolution.
  • Knowledge of community resources.
  • Ability to interact professionally with clients, coworkers, and community partners.
  • Ability to critically think and problem-solve while completing assessments and developing safety plans with clients.

Experience/Education:

  • Bachelor’s degree in Human Service field required, Master’s degree preferred (clinical psychology, psychology, criminal justice, educational psychology, rehabilitation psychology, guidance counseling, counseling psychology, social welfare, or social work).
  • Experience with treatment programs involving adults and youth with mental illness and/or substance use disorders.
  • One year of case management experience (preferred)

Title: Parenting Care Coordinator

Job Summary: This position is responsible for providing services within our Family Support area.  Care Coordinators provide one-on-one services that actively develop the skills necessary to parent, manage a home and/or maintain suitable housing as these conditions affect child safety. Services may include going into the home and observing the parent/child relationships, providing some level of supervision to the parent/child relationship or providing similar service involving the family as a unit. Care Coordinators provide an active, ongoing assessment of family stressors which affect safety and may result in necessary action.

Title: Youth Educator

Job Summary: This position is responsible recruitment of Milwaukee County youth who are enrolled in grades 6-8. Youth Educators will implement evidence-based programming that empowers teens with the tools and opportunities to build social-emotional skills, strengthen relationships, and avoid risky behaviors. Youth Educators will coordinate and collaborate with middle schools, youth serving organizations and project partners to implement program activities, community presentations and/or outreach activities. 

Job Duties/Responsibility: 

  • Facilitate youth groups that focus on building social-emotional skills, strengthen relationships, and avoid risky behaviors.
  • Plan, prepare and deliver lesson plans and instructional materials from the youth leadership multi-session curriculum.
  • Establish partnerships and collaborations with various schools, youth serving social service agencies and youth led councils for the recruitment of participants, implementation of curriculum, mobilization of youth led activities and creation of community coalitions for advocacy and support of gender violence prevention and youth leadership.
  • Identify and develop strategic community sites to implement and/or facilitate youth programming.
  • Participate in various community/school events, meetings, focus groups as identified by funders and/or program, and conduct community presentations and outreach to promote awareness of gender violence, healthy relationships, and youth leadership.
  • Complete and submit reports in a timely manner.
  • Attend scheduled meetings, conferences, workshops and community meetings.
  • Follow all requirements and guidelines of program funding under guidance of supervisors.
  • Performs special projects and other related duties as assigned.

Knowledge, Skills and Abilities:

  • High School Diploma or GED, supplemented by two years of post-secondary education, preferably in a human/social services field. One or more years of related work experience may be used to off-set minimum education.
  • At least one year of professional work or volunteer experience working with youth, implementing youth leadership programs and/or youth led community projects.
  • Demonstrated experience with facilitating groups that target youth in middle school, and/or at-risk youth.
  • Experience in public speaking, has strong writing skills, leadership, and organization skills.
  • Computer skills required, including ability to use of the current version of Microsoft office and/or Office 365 and
  • use system and data security protocols. Demonstrated knowledge and ability to work independently and as a team member.
  • Travel and work irregular/flexible hours on occasion.
  • Must have reliable transportation, valid driver’s license and auto insurance.
