Title: Parenting & Supervised Visitation – Care Coordinator

Position Type: Regular Employment (Part-Time)

Pay Frequency/Salary: Bi-Weekly

Schedule: Flexible; may include some evenings and weekends,

Reports to: Chief Executive Officer

Job Summary: This position is responsible for providing parents and children the opportunity to visit one another in a safe and supervised environment when children have been placed in out of home care due to abuse and/or neglect concerns. The family visit is constantly monitored to observe, supervise and document the visit of one referred family at a time. This position may be required to provide all transportation necessary to facilitate the visitation services including, when needed, transportation of the child(ren) to and from the visit. 

Job Duties/Responsibility: 

  • Arrange and supervise visits between children in temporary foster care placement and their families
  • Utilize knowledge of child development to conduct safe, supervised visits with family members
  • Monitor child/family interaction during visitation
  • Redirect and/or educate parents when needed on child engagement, discipline, nutrition, and basic care Safely transport children to and from visits, as needed
  • Intervene as an advocate for clients in emergency and crisis situations
  • Provide documentation within established timeframes
  • Transportation of children may be required
  • PLEASE NOTE: Visits may take place in office, community or in clients homes.

Knowledge, Skills and Abilities:

  • High School Diploma or equivalent (required) 
  • Associates Degree in Social Work or related field, Bachelor’s Degree in Social Work or a related field (preferred)
  • One year of case management experience (preferred)
  • One (1) years of experience working in the Social Service field (preferred)
  • Ability to communicate effectively and interact with a diverse population
  • Strong verbal and written communications skills as well as time management skills 
  • Organizational Skills
  • Computer skills including ability to utilize internet and complete accurate data entry.
  • Travel and work irregular/flexible hours on occasion.
  • Must have reliable transportation, valid driver’s license and meet auto insurance requirements.

Title: Youth Crisis & Mentor – Care Coordinator

Position Type: Regular Employment (Part-Time)

Pay Frequency/Salary: Bi-Weekly Pay

Schedule: Flexible; may include some evenings and weekends,

Reports to: Chief Executive Officer

Job Summary: This position is responsible for providing a one-on-one mental health-based intervention service with the primary purpose of responding to crises. A crisis should be responded to immediately or at least within 2 hours when an immediate response is not possible. This position also focuses strongly on skill building to increase positive communication and decision making as well as the teaching and modeling of behavior modification and de-escalation techniques needed to reduce negative behaviors and increase stability in the home and community. This position may require staff to visit schools to check on youth. Our agency strives to foster caring and supportive relationship with the youth we serve. This service is meant to develop the child’s goals for the future and help them meet their full potential in addition to assisting youth with creating a crisis/safety plan (if needed). This position provides guidance and support to address behavioral, social and emotional issues. Coordinators can be involved in a variety of activities with the client with a focus on social skills development, peer relationship building, personal care, goal setting and achievement, etc. Services need to focus directly on skill building.  May implement evidence-based programming that empowers teens with the tools and opportunities to build social-emotional skills, strengthen relationships, and avoid risky behaviors.

Job Duties/Responsibility: 

  • Positive interactions between the provider and child are required and are a key element in the success of these services. 
  • May facilitate youth groups that focus on building social-emotional skills, strengthen relationships, and avoid risky behaviors.
  • Plan, prepare and deliver lesson plans and instructional materials from the youth leadership multi-session curriculum.
  • Participate in various community events, meetings, focus groups as identified by funders and/or program, and conduct community presentations and outreach to promote awareness of gender violence, healthy relationships, and youth leadership.
  • Complete and submit notes in a timely manner.
  • Attend scheduled meetings, conferences and workshops.
  • Performs special projects and other related duties as assigned.

Knowledge, Skills and Abilities:

  • High School Diploma or GED, supplemented by two years of post-secondary education, preferably in a human/social services field. One or more years of related work experience may be used to off-set minimum education.
  • At least one year of professional work or volunteer experience working with youth.
  • Demonstrated experience with facilitating groups that target youth in middle school, and/or at-risk youth.
  • Experience in public speaking, has strong writing skills, leadership, and organization skills.
  • Travel and work irregular/flexible hours on occasion.
  • Must have reliable transportation, valid driver’s license and meet auto insurance requirements.
  • Bi-lingual in Spanish and English (both in verbal and written form) applicants encouraged to apply.
